Patient registration is an important aspect of running a medical institution. It can help track patients’ treatment, manage their medical history, and more.
So, a reliable patient registration form that helps you track people, analyze large amounts of data, and prepare personalized plans can be a great asset.
Unlike random-form generator tools, Google Forms, or other online forms, creating a form on your WordPress website can be a much better option.
On your website, you can create your own database, sort patient entries, and do much more. You have full control over the entries. However, you would also need the right registration plugin.
Why Use WP User Registration to Create a Patient Registration Form?
Different plugins have different features that make them unique. However, some features that make a registration form plugin stand out from the rest are mentioned below.
Variety in Form Fields: It’s really important to have the option to add as many form fields as necessary. This allows you to create diverse forms with all necessary fields included.
Easy Access: The form should be easy to access for both you, as the admin, and your users. It’s a bonus if the form analytics are well documented and presented in an easy-to-review process for the admin.
Tracking: The plugin should efficiently track and record the data submitted by the user using your form.
Registration Profile: Since we’re discussing a patient registration form, you need a plugin that enables you to create a unique user profile. This allows you to personalize the service you’re providing and makes it easier for users to track their progress.
Only a handful of WordPress plugins have the user profile feature. Moreover, it’s harder to find the plugin with this feature, and the ones mentioned above.

How to Create a Patient Registration Form?
Follow these five steps to learn how to create a patient registration form and use it on your website.
Step 1: Enable Users to Register on Your Website
By default, WordPress websites do not provide a registration option or save the data of the users that register on their website.
So, you must enable the Membership option before creating a registration form. Only when you enable the option will these registrations be assigned to a certain role within your website.
So, initially, you need to open your WordPress dashboard and go to Settings >> General. Then, you need to tick the “Anyone can register” checkbox.
Afterward, click the drop-down option for New User Default Role and select Subscriber. You can choose other roles for other forms, but Subscriber is the best choice for a Patient registration form.
Finally, scroll to the bottom of the page and click on Save Changes to save your changes.
Step 2: Download and Install the User Registration Plugin
Once you enable the membership option, you can move on to the next step. Now, you need to install and activate the User Registration WordPress plugin.
The free version of the plugin is available in the Add New Plugin section, and the pro version is available on the User Registration’s pricing page.
For Free Version
Open your WordPress dashboard and click on Plugins >> Add New Plugin.
Use the search bar to search User Registration. When the plugin appears in the search results, click on Install Now. When the installation is complete, click on Activate.
For Premium Version
Open User Registration’s pricing page and purchase a plan that fits your requirements. Once the purchase is complete, an email will be sent to your email address. You can open the mail and use the login information to open your own WPEverest account.
The account includes a zip file for User Registration Pro, all its add-ons, and its license key. You need to download User Registration Pro from the list.
On your website’s dashboard, hover over Plugins and click on Add New Plugin.
At the top, click on the Upload Plugin button. Then, click Choose File and upload the zip file you downloaded from WPEverest.
Next, click Install Now and then Activate Plugin once the installation is complete.
Again, go to the Plugins page by clicking on Plugins. There, locate User Location Pro and input the license key for the plugin.
The license key is available in the License Keys section of your respective WPEverest account.
Note: The free version of User Registration lets you build a versatile patient registration form. At the same time, the pro version has many more features that can help you elevate the quality of your form.
To access all these features, you need to install specific add-ons available with the pro plugin. The Install option will appear when you click on the locked field on your form builder.
Step 3: Create a Registration form
Once you’ve successfully installed and activated User Registration, you can begin creating the Patient Registration Form that’ll be perfect for your needs.
As we discussed earlier in the blog, you must decide which fields you want on your registration form. Once you have decided on your requirements for Form Fields, you can begin creating the form.
Hover over User Registration on your WordPress dashboard and click on Add New.
You can click on a prebuilt template named “Hospital Admission Form” and start customizing it or start from scratch.
Choosing the prebuilt option is much easier as it has almost all the form fields ready for your use. However, we’ll start this tutorial by creating a new patient registration form from scratch.
So, click on the Start from Scratch option, enter a name for your form, and press Continue.
The form builder’s left side contains form fields and field options, while its right side contains the form builder.
Step 4: Customize the Patient Registration Form
The new form includes fields for Username, User Email, User Password, and Confirmation Password. You need to add any other fields you want to add manually.
Initially, click on Add New to add a new section to the form.
Click on the grid option on the top right part of the section to choose the number of columns you want.
To add a new field, for example, an input field, you can click on the form field from the left and drag it to a suitable location on the form builder on the right.
Now, let’s customize the field. Click on the field that you added. Then, you’ll see the Field option for that field on the left. Then, manually change the sections under General Settings.
For example, add a name for your field under the Label section. Similarly, you can edit the Label, Description, Field Name, Placeholder, and other field settings.
Similarly, you can add other field options and customize them one by one.
After your customizations are complete, you need to click on the Update form button at the top.
Step 5: Show the Registration Form on Your Website
After you finish creating the ideal patient registration form, you can use it on any page on your website.
All you need to do is copy the form’s shortcode and paste it onto the page where you want the new patient forms.
The form’s shortcode is located at the top right of the form builder or in the All Forms section.
